Build input forms for your AI Flows with various question types.
Form Builder
If you are building a Form Flow, the form builder is where you design the input form your users will fill out. Every answer they provide becomes a variable that your AI workflow can use - so the form is the bridge between your users and your AI logic.
You can find the form builder in the Design tab of any Form Flow. It is the panel on the left side of the screen, next to the workflow canvas.
Adding questions
- Click the Add Question button at the bottom of your form.
- Choose a question type from the menu (see the full list below).
- Type your question text - this is the label your users will see.
- Configure any additional settings for that question (placeholder text, validation, and so on).
Question types
FormWise offers a wide range of question types to collect exactly the input you need:
Text inputs
- Short Answer - A single-line text field. Good for names, titles, or brief responses.
- Paragraph - A multi-line text area. Use this for longer responses like descriptions, instructions, or content briefs.
Choice inputs
- Multiple Choice - Users select one option from a list of choices. Displays as radio buttons.
- Checkboxes - Users can select one or more options from a list. Good when multiple answers apply.
- Dropdown - Users pick one option from a dropdown menu. Works well when you have many options and want to save space.
Number inputs
- Number - A numeric input field. You can set minimum and maximum values.
- Linear Scale - Users pick a value on a numbered scale (for example, 1 to 10). Great for ratings or priority levels.
- Range Slider - Users drag a slider to select a value within a range. A more visual alternative to the number field.
Date and time inputs
- Date Picker - Users select a date from a calendar. Useful for deadlines, event dates, or scheduling.
- Time Picker - Users select a time. Pair with a date picker for full scheduling.
Contact inputs
- Email - A text field with built-in email validation.
- Phone - A text field formatted for phone numbers.
Other inputs
- File Upload - Users can upload a file (document, image, or other supported formats). The file contents can be passed to your workflow.
- Rating - Users select a star rating (for example, 1 to 5 stars). A quick way to capture satisfaction or quality scores.
Configuring questions
Click on any question in your form to open its settings. Depending on the question type, you can configure:
- Required or optional - Toggle whether users must answer this question before submitting.
- Placeholder text - Hint text that appears inside the field before the user types anything.
- Help text - A short description that appears below the question to give users more context.
- Default value - Pre-fill the field with a value. Users can change it or leave it as-is.
- Validation rules - Set rules like minimum/maximum length, number ranges, or allowed file types.
Reordering questions
You can change the order of your questions at any time:
- Hover over the question you want to move.
- Click and hold the drag handle on the left side of the question.
- Drag it up or down to the position you want.
- Release to drop it in place.
Conditional visibility
Sometimes you only want to show a question if the user gave a certain answer to a previous question. For example, you might ask "Do you have a website?" and only show the "Website URL" field if they answer "Yes."
- Click the question you want to show or hide conditionally.
- Open the Conditional Visibility settings.
- Choose which previous question to base the condition on.
- Set the condition (for example, "equals Yes" or "is greater than 5").
- The question will now only appear when the condition is met.
Previewing your form
To see what your form will look like to users:
- Click the Preview button at the top of the form builder.
- A preview of your form opens so you can test it out and make sure everything looks right.
- Close the preview to return to editing.
How form data flows into your workflow
Every question in your form automatically creates a variable that you can use in your workflow. For example, if you have a question labeled "Topic," your workflow can reference that answer as a variable and include it in your AI prompt.
You do not need to set this up manually - FormWise handles the connection for you. When you configure an Agent node or any other workflow node, you will see your form variables available for use. To learn more about using variables, see Variables and Data.
